Acrefield House

Client Group and Admission Criteria

Acrefield House is registered to provide care for up to 12 adults who have a moderate to severe learning disability and who may have additional complex health conditions and sensory impairment. 

Admission to Acrefield House is primarily based on a 12 week assessment period which allows the multi-disciplinary team to undertake a detailed assessment of the individual service user’s needs in order to determine the most appropriate long term support plan.

The Service and Staff Team

Acrefield House provides a real sense of ‘home’ and a family feel for services users, many of whom have lived at Acrefield House for a long time. Whilst for some, placement at Acrefield may be an opportunity to develop living skills in readiness for more independent living, for others it will be a lifespan service, providing for an individual’s changing needs. Strong links with community health service providers ensures support with continuing health needs.

Individual care plans and risk management strategies will seek to maximise opportunities to ‘live life to the full’ and the contributions that family members and social contacts can make are welcomed and encouraged.  

Acrefield House is very much part of the local community and service users access a range of local amenities. Within the Home service users can enjoy arts and crafts, music therapy and a range of holistic therapies, such as massage and aromatherapy.                                                                                   

The staff team use Makaton and pictorial boards to overcome any communication difficulties that service users may have.

The staff team at Acrefield House receive comprehensive training specific to the needs of this particular group of service users. This is underpinned by the support from the clinical specialists within MHC.

The ground floor of Acrefield House is wheelchair accessible and provides all facilities needed to enable staff to support those with physical disabilities. 

Location

Acrefield House is a spacious, detached home with a large rear garden situated in a quiet area of Prenton, Wirral.   Where appropriate, the town provides all the amenities required on a daily basis including banks, small supermarkets, a post office, sports centre and a library. 

Prenton has good transport links via the motorway network, providing easy access to the cities of Liverpool, Chester and Manchester.
