Care pathways and plans

MHC recognises that each service user and patient is an individual in their own right with their own specific and often complex needs.

Our aim, through individualised care packages, is to provide the range of support needed to allow the person to achieve the most positive outcome for them.

That may be through a care pathway that returns them to independent living and employment or to a supported environment where they can access all the services the community has to offer but also benefit from staff support when it is needed.

To enable this progression, MHC offers step-down residential services to create opportunities for people to continue along their own care pathway at a pace that suits them.

Each person’s care pathway is continually monitored and appraised with regular reviews of progress and outcomes in partnership with the person themselves, care managers, family members, advocates and others. MHC works in close partnership with local authority and healthcare commissioners.
