Intensive Support

Overview

MHC's Intensive Support services provide the next level of support to assist people in making the transition from acute treatment environments, such as hospital services, to a more community based setting.  This will be at a pace consistent with the individual’s needs and wishes and their potential for progress.

Client Group & Admission Criteria

MHC’s intensive support, residential services provide a holistic model of care and support for male and female service users, aged 18-65.  Individuals admitted to these services will have a primary diagnosis of a learning disability and also displays behaviours that challenge.

The Service & Staff Team

The staff team is experienced in supporting individuals through their times of greatest need as well as helping them achieve their goals on a day-to-day basis.  Individuals are also supported by the Multidisciplinary clinical team to help provide stability during the times when it is most required.  The staff team will engage the service users in an individualised activity programme that will suit their needs and involve them in community participation through ordinary social activities.

Residential Services:

  • Holly - an 8 person, mixed gender, Intensive Residential Support Service.  Holly will aid the progress of the service user’s care pathways by establishing their needs and wants.  This helps in determining which type of support and environment would best meet the individual’s needs.  This could be a step-down into other Intensive Support services or a transition into another identified service.
  • Highfield House –  a home for 6 males with a mild – moderate learning disability and complex behaviours that challenge
  • Orchard – 4 single and 1 double apartment-style accommodation, for people with a mild – moderate learning disability who find it difficult to share accommodation due to clinical presentation or rehabilitative need
  • Rockfield – a 10 person, mixed gender service which provides highly specialized support designed specifically for adults whose behaviours have challenged other services to the point of breakdown
