Rehabilitation

Overview

Through a number of our residential support services, MHC aims is to rehabilitate service users to full community living by providing support and facilitating in the development of individual skills. This is achieved through risk management, developing a positive self image and increasing confidence. By helping to develop the individual’s network of friends and support, the service user is assisted to develop a fulfilling and meaningful life.

Client Group & Admission Criteria

MHC’s rehabilitation support residential services provide care and support for male and female service users, aged 18-65. Individuals who may be admitted to these services will have a primary diagnosis of a learning disability who also displays behaviours that challenge.

The Service & Staff Team

The services within MHC’s Rehabilitation Support stream offer therapeutic and structured programmes to assist in the rehabilitation process by enabling service users to access further education and meaningful work placements.

The staffing team is responsive to the changing needs of service users and will fluctuate accordingly to support those who need it. Our staff are highly motivated, passionate and experienced in dealing with the diverse needs of this client group.

Residential Services:

  • Larch – a 6-person, mixed gender home, providing care and support to service users who have a mild to moderate learning disability with some comorbid diagnoses. The home offers medium to long term residency
  • Rose – a 3-person home, that provides intensive residential support to individuals who have a learning disabilities and who may display complex challenging behaviour both to themselves and others
  • Pine – made up of 4 terraced Cottages that provide a service for up to four people, each in a homely, domestic environment
  • The Lodge – a 3 person, female home, accommodates service-users who have moderate learning disabilities and some comorbid diagnoses. It is a home for medium to long term residency
  • Avalon – A community based mixed gender service for up to 6 adults with a primary diagnosis of a learning disability and who may also have associated mental health problems
