Residential Care Home with Nursing

Glyn Rhosyn, a Residential Care Home with Nursing, is due to open in early winter 2010.  The service will operate as a graduated annex to St David’s Independent Learning Disability Hospital (MHC), enhancing the existing care pathway at the Hospital, as well as offering a pathway for those placed within other acute settings. 

Glyn Rhosyn will ensure service users have access to the most effective care and support in a purpose-built, community facing environment.

Introduction to Glyn Rhosyn

Glyn Rhosyn, a male only service, will specialise in the care and rehabilitation of 10 people, aged 18-65, who have a borderline to moderate learning disability.  The service will also support men who may have a co-existing secondary condition such as behavioural difficulties/challenging behaviours, mental health problems, autism, attention deficit hyperactivity disorder, epilepsy, and complex needs.

These individuals may require a step-down facility from acute services, such as Low/Medium Secure, Hospital, Assessment & Treatment services, or a step-up from community based services when a more robust, durable and sustainable nursing home provision is required for a period of care and rehabilitation. 

The home can provide care for service users who are not yet ready to return directly to their communities and who may be subject to conditions of guardianship or community treatment orders under the Mental Health Act 1983 (as amended 2007).

Flexible Packages of Care

Admission to Glyn Rhosyn is primarily based on an initial 12 week assessment period which allows the multi-disciplinary team to undertake a detailed review of the individual’s needs before determining the most appropriate long term support plan and care pathway.

The service also provides care managers and commissioners with the opportunity to repatriate service users, who are currently placed outside North Wales, closer to their homes, families and friends, whilst undergoing their care and rehabilitation. 

Care Pathway

Glyn Rhosyn offers a distinctive care pathway, with each step providing a clear progression towards the eventual aim of a community placement.  The person-centred nature of the service means that the care pathway is flexible to the needs of the service user. 

Individuals can be admitted to Glyn Rhosyn at any stage of the care pathway, and can step-up or step down, to make sure that their needs are met in the most appropriate settings depending on the levels of support and supervision they need. 

Service & Staff Expertise

Glyn Rhosyn’s philosophy of care is based upon the principles of care, rehabilitation, and social inclusion.   The service will be directly supported by St David’s highly experienced hospital management and multi-disciplinary clinical team comprising of:

  • Consultant Psychiatrist
  • Clinical Psychologist  
  • Occupational Therapist
  • Social Worker
  • Full time nursing and support team
  • Speech and Language Therapist
  • Activities Coordinator

Care, support and rehabilitation at Glyn Rhosyn will be organised through robust, focused assessment, planning, individualised interventions and continual review.  The service provides service users with a safe, person-centred environment where they can receive the appropriate care and support required to manage their complex and long-term needs. 

Location

Glyn Rhosyn is situated in Holywell town centre, Flintshire, and offers excellent access to local leisure and shopping amenities such as banks, post office, supermarkets, leisure centre, cafes and shops.

The home’s locality also provides excellent public transport links to larger urban areas such as Chester, Rhyl and Wrexham, which will ensure service users have access to meaningful rehabilitation in the domains of education, occupation and leisure.

St David’s Independent Hospital

St. David’s Independent Hospital specialises in the care, treatment and rehabilitation of male patients aged 18-65, who have borderline to moderate learning disabilities.  The hospital also supports individuals who may have a co-existing secondary condition such as behavioural difficulties/challenging behaviours, mental health problems, autism, attention deficit hyperactivity disorder, epilepsy and complex needs. 

St. David’s primary function is to rehabilitate patients leaving acute or secure services and to assist individuals when their current placement has broken down.   Patients can be admitted on an informal or formal basis under the Mental Health Act 1983.

Referrals

Referrals for Glyn Rhosyn and St David’s and are taken from a variety of sources including statutory Community Learning Disability Services, the Criminal Justice System, secure services, social services and healthcare professionals, independent social workers and solicitors.
