Learning Disability Service Delivery

We provide services for adults with complex and challenging needs who have experienced social exclusion. Through our multi-disciplinary team approach we actively promote care pathways and inclusive lifestyles which may, at times, require intensive therapeutic support and care.

Our Learning Disability Service offers quality care, support and treatment across our independent hospital and residential services. This dynamic process is underpinned by a multi-disciplinary team comprising of psychiatrists, psychologists, nurse specialists, social workers and occupational therapists who offer expert advice and support.

Through the comprehensive assessment process, which may include partnership work with other stakeholders, we deliver an eclectic range of support that is essential for our service users ensuring they are able to make choices about their care.

It is important that we empower service users to exercise their rights, independence, choice and inclusion. These philosophies are based upon partnership working with service users, families, advocates, carers and professional bodies.

The aim is to ensure the service affords adults with a learning disability the opportunities to have full, active and meaningful lives. We provide them with opportunities to help them develop a network of friends and create valued relationships. Maintaining existing and meaningful relationships is an important part of people’s lives. We ensure that contact is facilitated to meet their emotional and social needs.

We use specialists to drive forward the care delivery through robust clinical profiling, essential lifestyle plans, risk assessments and service user plans. This process is enhanced through staff training programmes that ensure theory to practice is delivered.

We are able to offer comprehensive care pathways for service users through our step-up and step-down facilities, maintaining a continuity of care until they are ready to move on.
