Rockfield House

Learning Disability

Rockfield is a spacious community based residential home for people living with a learning disability. The home offers intensive support using active support, person centred planning (PCP) and positive behavioural support (PBS) methods.

The home enables and supports individuals to enhance their living skills and work towards achieving independence and full empowerment.

Rockfield supports:

  • Males or females aged 18-65 yrs who have a diagnosis of a learning disability.
  • Support coexisting conditions including Autism, mental health, complex and challenging needs.
  • Admission is primarily based upon an initial 12 week assessment period.

Philosophically, we believe in understanding the needs of the individual through robust assessment in order to provide person centred support to enable individuals to lead fulfilling lives as part of their local community.

We do this through:

  • Psychologically informed environments that encourage staff to reflect on practice and how to support the individual.
  • Person centred plans to develop positive behaviour support, enabling individuals to maintain and develop skills leading to greater independence.
  • Providing services that represent good value for money by demonstrating measurable, effective outcomes and progression.
  • Ensuring our colleagues have opportunities for tailored training and development to meet individual needs.
  • Ensuring that compliance and audit systems support service delivery and constantly drive improvement and embed quality in everything we do.
  • We work with a wide range of stakeholders to meet the needs of the individuals needs.

Rockfield provides a culture and environment that empowers individuals to fulfil their potential. By working together, with the individual, commissioners and families, we are able to develop pathways that reflect individual ambitions and aspirations. Our core ethos is to support people to achieve positive outcomes.

All residents have individual plans combining both areas of recreational and therapeutic input, as well as a daily programme of activity which encourages decision making where possible. Integration into the local community is a fundamental part of each residents care plan. Activities include ten pin bowling, disco, cinema, walking, swimming, meals out and snooker club to name a few.

Staff utilise all available skills, resources, support models and research to progress individuals towards achieving positive outcomes.

By enabling staff to understand the function of risk behaviours this enables plans to be implemented to safely manage potential risks and to provide the least restrictive support in an inclusive and empowering way.
